NOTIFICATION AND CANCELLATION AGREEMENT WITH PATIENT
COSMETIC APPOINTMENTS
All cosmetic appointments are held by credit card taken at the time of appointment booking to store in our secure eWallet system. Payment in full will be due at the time of your appointment and can be made with credit card, cash, or check.
LATE ARRIVALS
Late arrivals could force provider to either reschedule you or see you for a shorter period of time. If the provider is unable to see you at your arrived time, then you will be billed based on the Cancellation Fee listed below. If the provider determines s/he can see you at your reduced time slot, then you will forfeit the additional time you would have had if you arrived on schedule and no money will be refunded.
EARLY DEPARTURES AND NON COMPLIANCE
If you decide to leave your appointment time early, for whatever reason, then no part of the appointment charge will be refunded to you. If it is determined that you are non-compliant and are unable to be treated, then you will be billed based on the Cancellation Fee listed below. No part of the appointment charge will be refunded to you.
FEES CHARGED
A fee will be charged for any appointment that is not cancelled or rescheduled within the time period listed below. This includes No Shows (i.e., people who do not arrive for their scheduled appointment time), Late Arrivals, and Non-Compliance.
